Lack of testing is why nobody knew the scope of it. India has only tested 4,102 per million population, by far the lowest testing rate of any major country. India ranks #138 in the world on testing rates.

But with the slums of Mumbai being a massive virtual incubation chamber, it had to spread though the slums like wildfire. I'm sure thousands upon thousands died in the slums that weren't accounted for at all.
